1. Medical Services Disclaimer
Found Wellness provides telehealth and wellness services. Information on this website is for educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Establishing care requires a formal patient-provider relationship through a scheduled consultation.

2. Telehealth Services & Location
Medical telehealth services are provided only to patients physically located in the state of Nevada at the time of their visit, where the provider is licensed as an APRN, FNP. Lactation support services are available nationwide in all 50 states through certification as a Certified Lactation Counselor (CLC) and are educational in nature.

3. No Emergency Services
Found Wellness does not provide emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

4. Membership & Services
Membership provides ongoing access to care, including follow-ups, lab review, treatment adjustments, and communication between visits. Membership requires a minimum 3-month commitment with monthly auto-pay. Membership does not include the cost of labs, medications, or supplements unless otherwise specified. Services are non-transferable.

5. Payments & Refund Policy
All services are cash-pay only and payment is required at the time of booking or service. Consultation fees are non-refundable. Membership fees are non-refundable once billed. Lab testing, medications, and treatment plans are non-refundable once ordered. IV therapy and injectable services are non-refundable once administered.

6. Lab Testing & Results
Advanced testing (including GI-MAP, hormone panels, and specialty labs) is used to guide care. Results are interpreted in the context of your overall health and do not guarantee specific outcomes. Patients are responsible for completing recommended labs in a timely manner.

7. Medication & Treatment Disclaimer
All treatments, including hormone therapy, peptide therapy, NAD+, and weight loss medications, are prescribed based on clinical judgment and individual needs. Results may vary. Patients agree to follow instructions and report any side effects. Found Wellness is not responsible for misuse of prescribed medications.

8. NAD+ and Injectable Therapies
Programs such as NAD+ therapy are provider-guided. Patients must follow all storage, handling, and dosing instructions. Once opened, compounded medications should be used within recommended timeframes per pharmacy guidelines.

9. Communication
Membership may include messaging access. Response time is typically within 72 business hours. Messaging should not be used for urgent or emergency concerns.

10. Privacy
Your personal and medical information is handled in accordance with applicable privacy laws. Please refer to our Privacy Policy for more details.

11. Limitation of Liability
To the fullest extent permitted by law, Found Wellness shall not be liable for any indirect, incidental, or consequential damages related to the use of our services or website.

12. Changes to Terms
Found Wellness reserves the right to update these Terms and Conditions at any time. Continued use of services constitutes acceptance of any changes.
